I hereby waive all claims against the Hemophilia Foundation of Greater Florida its sponsors, or any personnel for any injury I might suffer in this event. I attest that I am physically fit and prepared for the event. I grant full permission for organizers to use photographs of me, and quotations from me, in legitimate accounts and promotions of this event.

For Bleeding Disorder Walk participants under 18 years of age, parental permission is required. You must have written consent from your parent or guardian. Team Captains are responsible for verifying this information with the HFGF, as well.

The Hemophilia Foundation of Greater Florida will not be providing childcare during the event.  All children must be accompanied by a parent or guardian.
